Join our Team; Triage Nurse
An opportunity has arisen for a Triage Nurse to join our expanding community team. This role will act as a first point of contact for all new referrals to our service ensuring that patients receive the most appropriate care from the right person at the right time.

St Vincent’s Hospice is a specialist palliative care provider with an 8-bed In-Patient Unit, a Day Services Hub at the hospice, and five retail stores across Renfrewshire. Our mission is to provide compassionate, high-quality care and support to individuals and families affected by life-limiting conditions.
Your Role & Responsibilities
The successful post holder will be responsible for gathering information on disease status, treatment plans, past medical and medication history and then making a decision regarding onward referral to the most appropriate part of St Vincent’s clinical team. This post holder will be required to give specialist advice on symptom management to patients, families and health and social care colleagues. The post holder may, on occasion, visit individuals in their homes in order to carry out first assessments and initiate treatment plans for symptom management. There will be a significant element of liaising with our health and social care colleagues in order to achieve the best outcome for individuals and those important to them.
The post holder will have in-depth knowledge of all parts of the clinical service at St Vincent’s in order that referral to the most appropriate department is made. To achieve this it will be necessary for the post holder to be clinically familiar with all areas which includes Community Services, Family Support Services, In Patient Unit, Day Services and our Supportive Care Clinic.
The successful post holder will be a Registered Nurse with experience in palliative care and/or supporting those with life limiting illnesses. They will possess excellent assessment and organisational skills. They will demonstrate advanced communication skills including being able to adapt these skills according to the situation. They must be able to work flexibly and continually respond to the demands of a clinically dynamic caseload.
Working Hours:
This is a full-time post, working 37.5 hours per week (weekend availability) and in return you will receive a competitive salary of £34,056 per annum.
